Re-register a birth
You can re-register a birth if:

the parents have married after the child was born and you want the natural father’s details to be on the birth record
-
you are not married and want the father’s name on the birth record
If the child is 16 years old or over, you need their written consent to change their last name.
How to re-register a birth
To apply to re-register a birth, you need to find the form that fits your circumstances on GOV.UK.
Complete the form and either send it to us by post or deliver it in person to our register office at:

You need to include any relevant original documents such as the child’s existing birth certificate and marriage certificate (if applicable). Include a translation if it’s in a language other than English.
After you apply
When we receive your application, we’ll book a re-registration appointment for you.
In some cases, such as foreign marriages or court orders, we’ll have to ask the General Register Office for approval. This can take up to 6 weeks.
To book a re-registration appointment, you must pay a £25 non-refundable fee. This includes 2 birth certificates when you attend the appointment. If you do not attend your appointment, you will not be refunded.
